What Is Perimenopause? Common Signs Every Woman Should Know

Many women are surprised when symptoms like poor sleep, fatigue, headaches, or mood changes begin appearing in their 40s.

They may wonder if they're under too much stress, working too hard, or simply getting older.

What many don't realize is that these changes may be part of a natural stage of life called perimenopause.

One of the most common things I hear from patients is:

"I just don't feel like myself anymore."

If that sounds familiar, you're not alone. Understanding what perimenopause is—and recognizing its early signs—can help you better understand what's happening and have informed conversations with your healthcare provider.

What Is Perimenopause?

Perimenopause is the natural transition leading up to menopause. During this time, the ovaries gradually produce less estrogen and progesterone.

Rather than declining steadily, hormone levels often fluctuate from month to month. These fluctuations can lead to a variety of symptoms, which is why many women notice changes long before their periods stop completely.

Perimenopause ends when you've gone 12 consecutive months without a menstrual period. At that point, you've reached menopause.

For many women, the transition lasts several years.

When Does Perimenopause Begin?

Most women begin perimenopause sometime during their 40s, although some notice changes in their late 30s while others don't experience symptoms until their late 40s or early 50s.

There is no single "right" age.

Every woman's experience is unique, and symptoms may come and go over time.

Common Signs and Symptoms of Perimenopause

Perimenopause can affect much more than your menstrual cycle.

Common symptoms include:

  • Changes in menstrual cycles

  • Hot flashes

  • Night sweats

  • Difficulty falling or staying asleep

  • Fatigue or low energy

  • Mood changes or increased irritability

  • Brain fog or difficulty concentrating

  • Headaches or migraines

  • Vaginal dryness

  • Changes in libido

  • Joint aches or muscle stiffness

Some women experience only a few of these symptoms, while others notice several at the same time.

Why Do the Symptoms Seem So Unrelated?

One reason perimenopause can be confusing is that the symptoms don't always appear together.

You might first notice that you're sleeping poorly.

A few months later, headaches become more frequent.

Then you begin forgetting small things or feeling unusually irritable.

Because these changes develop gradually, many women assume they're simply stressed, overworked, or aging.

When viewed together, however, they often tell a different story.

When Should You Talk With Your Healthcare Provider?

Although perimenopause is a normal stage of life, it's still important to discuss new or concerning symptoms with your healthcare provider.

You should seek medical evaluation if you experience:

  • Very heavy menstrual bleeding

  • Bleeding between periods

  • Severe pelvic pain

  • Significant mood changes

  • Symptoms that interfere with your daily life

Other health conditions—including thyroid disorders, anemia, sleep disorders, and depression—can cause symptoms similar to those experienced during perimenopause. A proper evaluation can help determine what's contributing to the way you feel.

Where Can Acupuncture Fit In?

Many women choose acupuncture as part of a comprehensive approach to supporting their health during perimenopause.

At Spirit House Medicine, treatment begins by understanding the whole person—not just one symptom.

How are you sleeping?

How are your energy levels?

Have your headaches changed?

How is stress affecting your daily life?

By looking at the bigger picture, treatment can be individualized according to your symptoms, health history, and goals while complementing the care you receive from your healthcare team.

Looking Beyond Hormones

Perimenopause is about much more than changing hormone levels.

It can affect how you sleep, think, feel, and participate in everyday life.

You may find yourself too tired to enjoy time with your family, struggling to concentrate at work, or wondering why you don't quite feel like yourself anymore.

The good news is that you don't have to navigate these changes without support.

Understanding what's happening is the first step toward making informed decisions about your health and finding the care that's right for you.
